Cultures FIRST
Send all relevant cultures before starting antibiotics whenever possible
Review at 48–72h
De-escalate or stop based on culture results. Narrow the spectrum.
Renal Adjustment
Check eGFR/CrCl for every antibiotic. Adjust doses accordingly.
